Aorto-Uni-Iliac Stent Grafting and Femoro-Femoral Bypass in a Patient with a Failed and Catastrophic Endovascular Aortic Aneurysm Repair

چکیده
A 78-year-old man presented at Eulji University Hospital due to an abdominal aortic aneurysm with maximum diameter of 52 mm, which had been increased from 45 mm over 6 months. He underwent embolization of the left internal iliac artery with vascular plug, prior to endovascular abdominal aortic repair with a bifurcated stent graft system. Unfortunately, the inserted vascular plug was maldeployed and protruded into left external iliac artery, and caused acute limb ischemia. Because revascularization of the occluded segment was failed, emergent hybrid approach with aorto-uni-iliac stent grafting and femoro-femoral bypass was done, successfully.

PURPOSE To report a case of type IIIb endoleak developed six years after endovascular abdominal aortic aneurysm repair (EVAR). CASE REPORT A 75-year-old man underwent successful Talent™ stent-graft positioning to treat a 53 mm abdominal aortic aneurysm. Subsequently the patient did well and yearly routine control computerized tomography (CT) was unremarkable. Six years later the patient sudde...
متن کاملEndovascular aortic aneurysm repair using a bifurcated stent graft in a patient with femoro-femoral bypass
A challenging iliac anatomy remains a major obstacle to successful endovascular aortic aneurysm repair. The authors experienced an abdominal aortic aneurysm with bilateral common iliac artery aneurysms in a patient that had undergone femoro-femoral bypass due to left external iliac artery (EIA) occlusion. To treat this patient in a totally endovascular manner, a bifurcated stent graft was used....
متن کاملBilateral prevascular herniae – A rare complication of aorto-uniiliac endovascular abdominal aortic aneurysm repair and femoro-femoral bypass
A case of a 78-year-old female with bilateral groin prevascular herniae following an emergency aorto-uniiliac EVAR and femoro-femoral bypass for a ruptured abdominal aortic aneurysm is presented. Primary repair of the herniae was achieved using a preperitoneal approach. The case emphasises a safe approach to dealing with this rare complication.

Three randomised controlled trials have shown a significant reduction in peri-operative mortality when comparing endovascular aneurysm repair (EVAR) with standard open surgery (1.7% v. 4.7% in the EVAR Trial 1; 1.2 v. 4.6% in the Dream Trial; 0.5 v. 3% in the OVER Trial1-3). EVAR has therefore become the standard of care in many centres. There are, however, patients who require EVAR because ope...

A 78-year-old male presented with early gastric cancer and a 5.5 cm-sized infrarenal abdominal aortic aneurysm, detected during regular screening. Endovascular aneurysm repair (EVAR) was performed first, followed by laparoscopic distal gastrectomy. After gastrectomy, the patient underwent computed tomography (CT) scan due to persistent fever, which showed increased perigraft fluid collection ar...
